Overview

We are seeking a dedicated and highly skilled Registered Nurse (Management) Care Manager to join our healthcare team. Duties

  • Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs.
  • Develop and implement personalized care plans in collaboration with physicians, specialists, and other healthcare professionals.
  • Manage clinical case management activities, including discharge planning, post-acute care coordination, and home health services.
  • Utilize electronic health records (EHR) systems to document patient information accurately while adhering to HIPAA privacy regulations.
  • Oversee utilization management processes by evaluating diagnostic evaluations, managing hospital admissions and discharges, and ensuring appropriate resource use.
  • Provide patient education on health conditions, medication management, and preventive care strategies to promote self-care.
  • Collaborate with multidisciplinary teams to optimize patient service delivery across inpatient, outpatient, emergency room, and clinic settings.
  • Monitor patients' physiological parameters and vital signs regularly to detect changes requiring intervention.
  • Engage in medical records review and documentation to support clinical decision-making and regulatory compliance.
  • Participate in quality improvement initiatives focused on infection control, hospital medicine standards, and behavioral health integration.

Experience

  • Proven experience as a registered nurse with a strong background in critical care environments such as ICU or Level II/Level I trauma centers.
  • Extensive hospital experience across medical-surgical units, emergency medicine departments, or specialized units like PICU or geriatrics.
  • Demonstrated expertise in managing complex cases involving infectious diseases, post-acute care needs, or hospice services.
  • Familiarity with EMR/EHR systems for efficient documentation and data retrieval.
  • Knowledge of health regulation policies including HIPAA compliance and medical terminology standards.
  • Prior experience in case management roles within hospital medicine or home health settings is highly desirable.
  • Strong communication skills for client interaction and interdisciplinary collaboration.
  • Ability to perform comprehensive patient assessments and diagnostic evaluations effectively.
  • Experience managing patients in inpatient and outpatient environments with a focus on discharge planning and post-discharge follow-up.
  • Understanding of managed care principles, utilization management processes, and primary care coordination.
